Hair Loss

Hair loss affects 38 million men and 21 million women. It can be a very stressful development for both men and women; balding changes our appearance and can undermine our confidence.

Pattern baldness, also known as alopecia, is the most common cause of hair loss. The condition will affect about 70% of men and nearly 40% of women at some point in their lives. Typically, for men, baldness begins with a hairline recession that starts at the temples. For women, hair density thins over the top of the scalp. There are many factors that contribute to pattern baldness, including genetics and environmental factors.

Hair Transplants

Statistics compiled by the International Society of Hair Restoration Surgery (ISHRS) show hair restoration surgery increased 47.9% since 2008. In general, hair transplants involve harvesting hair from a healthier donor site of the patient and implanting hair follicles into an area that is lacking. Hair transplantation may also be used to restore hair loss on the eyelashes and eyebrows.

Types of Surgical Hair Restoration

There are two general types of techniques that are used for surgical hair transplantation: the “strip method” and Follicular Unit Extraction (FUE) with NeoGraft™. Both techniques involve moving healthy hair follicles from a donor site of the scalp to a bald or thinning area of the scalp.

NeoGraft™ FUE

NeoGraft™ FUE is the type of hair replacement therapy favored by Dr. Robbins at Des Moines Plastic Surgery™. NeoGraft™ is the new method of follicular hair transplantation with no scars and minimal downtime. NeoGraft™ FUE uses a tiny 1 mm punch to gently remove individual follicles from the donor site. A single graft contains from 1 to 3 individual hair follicles. The transplanted hair follicles using NeoGraft™ are natural and grow just like normal hair.

Who is a good candidate for FUE using NeoGraft™?

Patients who would like the option of wearing a very short haircut in the back or sides of their scalp, or those who want the least amount of activity restriction (e.g. athletes) after their hair transplant procedure, may be good candidates for FUE with NeoGraft™. FUE can also be used for scar camouflage procedures for patients who have had prior strip method procedures, as well as for body hair transplants.

Ideal candidates for this treatment have realistic expectations for hair-regrowth results. To help set proper expectations, it is best to look over NeoGraft™ before and after photos.

Is FUE/NeoGraft™ suitable for both men and women?

Yes. Because FUE/NeoGraft™ is a minimally invasive procedure, it is a viable option for both men and women looking to restore their own living and growing hair with transplantation.

What is the NeoGraft process like?

Dr. Robbins will perform NeoGraft™ hair transplantation using local anesthesia. The NeoGraft™ handpiece gently extracts individual micro hair grafts from hair bearing scalp. Each harvested graft is then placed into thinning or balding areas of the head. Once the transplant is complete, you will be able to return home and can resume your usual activities within a few days.

After about 2 to 3 weeks, the transplanted hair will fall out, but the transplanted hair follicle bulbs will remain under the skin. After 4 to 6 months, you will notice significant hair growth, with final results evident after about 9 months to 1 year.

How successful is NeoGraft™ hair transplantation?

How much hair can be grafted?

250-3,000 grafts in a session. 2,500 grafts will cover an area about the size of a CD. More than 2,500 grafts can be grafted but would require more than one session.

How long does the procedure take?

It depends on the number of grafts. 500 grafts take about 4 hours. 2,500 grafts take about 8 hours (4 hours to harvest the hair and 4 hours to implant). Patients can read, listen to music, watch TV, or use their computer. For longer cases, the patient will be provided a lunch break.

How long does it take to see hair growth?

“Shock loss” sometimes occurs after treatment, and the new hair will grow back within around 3 to 4 months. In about 9 to 12 months, the new hair will begin to grow normally. It takes this long because of the natural slow growth process of hair.

When can I return to work?

A few days off is typical, but some patients return the next day to work and social activities. Strenuous activity should be avoided for a week.

The Strip Method

The strip method is the more common of the two general techniques and is also the more invasive option. The basic idea behind the strip method is that one or more strips of skin are excised from a part of the scalp where hair growth is healthy. Generally, the strip will be 1-1.5 cm in width and 15-30 cm in length.

As the incision site is sutured, assistants dissect individual follicular units, which are small groups of hair follicles. Great care is taken to avoid damaging the hair follicles during this step.

As for the incision, the latest method, trichophytic closure, is used to hide scarring. Closure has been used by plastic surgeons for decades for cosmetic incision lines. Traditional closure is simply a suturing of two sides, but the trichophytic method takes an extra step to trim the incision site for a pattern that allows the hair to grow along the edge of the incision.

For the final steps of transplantation, micro blades or fine needles are used to puncture sites for receiving donor hair follicles. Technicians then insert grafts individually into place. The patterns are predetermined and planned to create a natural growth pattern. Angling of the punctures also adds to a more natural appearance.

Strip Method Recovery

The recovery period for the strip method is generally two weeks for the incision in the donor site to heal. The size of the donor strip depends on the specifics of the patient, such as the patient’s hair density. Hair density in the back of the scalp ranges from 60-100 follicular units per square centimeter, which is generally going to be denser than on the side of the scalp.

For best results, follow the post-surgical care directions of Dr. Robbins diligently. During the initial period of healing, the transplanted hairs will fall out due to “shock loss” because of surgical trauma. Within two to three months, the new hair will grow back, and it will continue to grow and thicken through the next six to nine months.

Strip Method Cost

The exact price of a hair transplant using the strip method is determined by the specific details of the case. Strip method cost is impacted by the number of grafts used, the number of strips created, and the overall time required to perform the transplantation procedure.

NeoGraft™ vs. Strip Method

There are similarities between both options. Both techniques result in the transplantation of healthier hair, which will continue to grow in the new site for the rest of your life. With both techniques, the pattern for placement is planned for the most natural results.

The basic idea of implantation of follicle grafts for both techniques are similar. The hair follicles are implanted into the donor site by using micro blades or fine needles to puncture sites for the follicle graft to be placed. With the NeoGraft™ technique, this step is automated.

The main differences are the method involved with harvesting hair from the donor site and the consequences of said method.

The strip method involves making incisions in the scalp to remove a strip of hair-bearing skin from the back of the head and then transplanting the individual hair follicles in the non-hair-bearing areas. The procedure works well, but many patients object to the large scar in the harvest site. The scar can be visible and painful, and it can dictate how short the patient’s hair is worn for fear of showing the large scar. The strip method is also invasive, requiring stitches and a prolonged recovery period.

The NeoGraft™ technique avoids the need for removing strips of scalp from a donor site and instead uses a minimally invasive technology that targets hair follicles from a donor area and quickly extracts them. The device is handheld and is the only system on the market that is a complete, automated harvest and transplant system with “no touch” implantation technology. NeoGraft™ uses controlled pneumatic pressure to slide out hair follicles.

The overall recovery time is shorter with NeoGraft™ and is said to be more comfortable due to the lack of the linear incisions involved with the strip method. This technique has less bleeding, no excision of skin, and no sutures.
